A household is eligible if one member of the household meets at least one of the criteria below:
Has an income that is at or below 135% of the Federal Poverty Guidelines. The guideline is based on your household size and income.
Participates in certain assistance programs, such as SNAP (formerly known as Food Stamps), Medicaid, Supplemental Security Income (SSI), Federal Public Housing Assistance, or Veterans Pension and Survivors Benefit.
Participates in Tribal specific programs, such as Bureau of Indian Affairs General Assistance, Tribal TANF, Head Start (only households meeting the income qualifying standard), or Food Distribution Program on Indian Reservations.
If your child or dependent participates in any of the programs listed above.
Some providers may have an alternative application that they will ask you to complete. Eligible household must apply for the program and contact a participating provider to select a service plan.
